A l'origine des grandes révolutions scientifiques, il y a une idée philosophique de l'homme : l'" animal rationnel " de l'Antiquité est lié à la naissance des sciences naturelles ; à l'âge classique, l'" âme étroitement unie à un corps " de la métaphysique cartésienne est indissociable de la physique mathématique ; le "sujet assujetti" du structuralisme était l'objet privilégié des sciences humaines triomphantes du siècle passé ; et le vivant défini par ses " capacités cognitives " marque la victoire actuelle des neurosciences. Chaque définition de l'homme charrie aussi son lot de croyances morales et d'idéologies politiques, d'autant plus puissantes qu'elles semblent soutenues par les certitudes scientifiques de leur époque. Derrière l'esclavagisme ou le racisme, à l'origine du totalitarisme ou des formes les plus subtiles de l'antihumanisme contemporain, se trouve une définition de notre humanité. C'est toujours au nom de ce qu'est l'homme ou de ce qu'il doit être que l'on prescrit ce qu'il faut faire et ne pas faire. Cuando era un joven cirujano plástico, recuerdo bien cómo el interior de la cara parecía un territorio misterioso y peligroso donde todo lucía igual y solo la obediencia al ritual empírico del lifting facial me permitía sentirme seguro. Con el tiempo, los cambios cada vez más sutiles de la anatomía se convirtieron en buenos compañeros para la cirugía. Contemplé los ligamentos de retención de la cara, delgados y débiles individualmente, pero muy poderosos en conjunto. Más adelante, comencé a entender cómo la anatomía contribuía tanto a la apariencia de la cara como a la manera en que esa comprensión podría ser una herramienta para entender la cara envejecida y mejorar las formas para rehabilitarla. El conocimiento es poder. Al reemplazar o agregar volumen a la cara envejecida, las sutilezas de la colocación pasaron de ser ignoradas a ser obvias. Al principio uno está feliz de que un área se pueda llenar. Con el tiempo, los poderes de observación y discriminación aumentan hasta el punto en que los intentos anteriores se ven toscos e ingenuos y uno observa y se esfuerza por obtener resultados más agraciados y menos exagerados. Casi todas las modificaciones estéticas de la cara se basan en la anatomía, cuyos detalles suelen ser la última consideración del médico estético. Los liftings faciales se realizaron durante aproximadamente seis décadas antes de que se entendiera la anatomía del nervio facial. Los primeros años en el uso de rellenos fueron en su mayor parte para rellenar arrugas. De ese modo, se podría concluir que la adopción comercial inicial de una técnica impide el estudio cuidadoso de su ciencia subyacente. Pero las cosas han cambiado: hemos crecido en el conocimiento de muchos aspectos de la cara. El estudio de la anatomía facial ha florecido en los últimos años; al prestar más atención a las áreas de interés perceptual, se puede ver que las estructuras que definen los rostros de diferentes edades se han modificado de manera comprensible, no misteriosa ni arbitraria. Los doctores Lamb y Surek nos han proporcionado un notable volumen sobre anatomía facial, compilando información de otros autores pero también incluyendo extensas disecciones limpias y descripciones propias. Los diagramas, las mejores herramientas explicativas, son particularmente valiosos por su concisión y corrección y porque abordan las preocupaciones del médico. Los fenómenos anatómicos de la cara, las concavidades, las ondulaciones, los montículos, entre otros, revelan que tienen sus orígenes en lo profundo de la cara de forma que no son fáciles de apreciar. Los tratamientos se pueden hacer de manera más racional con tales entendimientos y muchos se incluyen en este volumen. El conocimiento es poder. Este libro es un importante paso adelante para comprender no solo la anatomía de la cara, sino cómo funciona y cómo se puede modificar para bien. Cualquier estudiante de la cara se beneficiaría mucho de estudiar este tema y este libro en particular.
